Meaning
was supposed to… (but didn't happen)
〜はずだった means something was expected or scheduled, but reality turned out differently: 九時に出発するはずだったが、遅れた "it was supposed to leave at nine, but was delayed". It is the past of はずだ ("should be") and almost always continues with が / のに and the disappointing fact.
Talking about plans, timetables, arrangements and predictions that failed. The subject can be anything (a plane, a person, the weather). For your own intention use つもりだった.
