it is the rule/arrangement that ...
〜ことになっている means "it is the rule / the arrangement that…". It reports a rule, custom, schedule or decision made by someone other than the speaker: the meeting is held every Monday; phones may not be used in school; I am set to meet the manager tomorrow.
Both registers; very common in explanations of rules and appointments. It follows Vる or Vない. Compare 〜ことにしている, which is the speaker's own habit or decision. Because the decision belongs to others, it sounds objective and slightly distancing.
